Covid-19 Vaccination Survey for Tribal Community
The Mashpee Wampanoag Tribe Emergency Management Department (MWTEMD), in collaboration with the Centers for Disease Control & Prevention (CDC) and the Mashpee Health Service Unit (IHS), have sent out two separate vaccination surveys.
One survey is for individuals 12 years and older (General Vaccination Survey), and the other survey is specifically for parents and guardians with children 5 – 11 years of age (Pediatric).
We would like to hear your thoughts on COVID-19 vaccinations. The surveys are anonymous, with no identifying information collected. Each survey will take less than 5 minutes to complete. Thank you for taking the time to share your thoughts with us. We ask that you complete your survey(s) by Monday, November 22nd at 5:00 PM EST.
